There are several possible locations for the small incision that will be used for inserting the breast implant. The most frequent technique utilizes an incision made in the lower portion of the breast. Another technique, though less frequently used, involves making an incision in the armpit. A third technique makes an incision around the areola (the darker skin surrounding the nipple). A newer technique uses an incision in the belly button, thus reducing the chance of scarring in the breast area. The best technique will be decided together between the patient and the surgeon during the consultation.


During surgery, the breast tissue is raised to create an open pocket under the breast tissue or beneath the chest wall muscle. Inserting an implant behind each breast can increase a woman's breast size by one or more bra cup sizes. Implants typically contain a saline solution (similar to saltwater) although in special cases the implants can contain a silicone gel. In some circumstances, particularly those in which there is breast asymmetry (uneven breast size), an inflatable implant may be used to allow the surgeon to adjust the level of inflation to attain breast symmetry and balance. Surgery typically lasts from 1 to 3 hours.

Generally, post-operative instructions call for plenty of rest and limited movement in order to speed up the healing process and recovery time. Bandages are applied right after surgery to aid the healing process and to minimize movement of the breasts. This pain can be treated effectively with oral medication. Breast reduction without lift- When the skin still retains good elasticity and the severity of the sag is minimal, it is possible to simply remove some of the excess skin and fat without performing a lift.


Breast reduction with lift- When the breasts are extremely large and sagging is severe, a lift is performed in conjunction with the breast reduction. It is also necessary to reposition the nipple to a natural position on the higher placed breasts. There are several different incision choices depending on the extent of the reduction: crescent incision, periareola incision, keyhole or Lejour incision and the anchor incision, which is the most common.


Breast reduction using liposuction- This is the typical method for male breast reduction and can sometimes be used for women if the amount of tissue removal is minimal. Once the fat is vacuumed out, the skin bounces back to shape. This method is sometimes used to obtain symmetry if there is slight asymmetry present with one breast or the other with very minimal incision.


Once the excess tissue is removed, patients will experience a lightening of the torso area. Stress and strain to the neck and back will stop. Chronic pain issues can be reversed and of course, there is a renewed sense of self-confidence.

We routinely do not perform this procedure on anyone less than 18 years of age?

New Technique Doubles Breast Size Using Patient's Own Fat

A plastic surgery procedure in which the patient's own fat is transplanted to the breasts?used along with treatment to expand the breast tissue before surgery?can achieve up to a twofold increase in breast size, according to a study in the June issue of Plastic and Reconstructive Surgery?, the official medical journal of the American Society of Plastic Surgeons (ASPS).


The procedure builds on previous fat transfer techniques to provide excellent outcomes of breast enhancement surgery. "Pre-expansion to the breast allows for mega-volume (over 300 cc) grafting with reproducible, long lasting results that can be achieved in less than two hours," according to ASPS Members Daniel Alexander Del Vecchio, MD, Boston, and Louis Paul Bucky, MD, Philadelphia, authors of the new report.


"Pre-Expansion" Maximizes Results of Breast Fat Transfer

The technique is an adaptation of the increasingly popular autologous fat transplantation technique. In these procedures, fat obtained by liposuction from one part of the body?for example, the thighs?is transferred for use in breast enlargement and reshaping.


In the procedure used by Drs. Del Vecchio and Bucky, the patient first undergoes several weeks of "pre-expansion" treatment. This is done with a bra-like device that uses gentle negative pressure to gradually expand the breast. The pre-expansion procedure provides extra room in the breast, which is "backfilled" using the liposuctioned fat.


The authors report their experience using pre-expansion and autologous fat transfer in 46 breasts of 25 women. Some of the patients wanted to increase their breast size or to replace implants; others were seeking treatment for certain types of breast deformities. On average, about 300cc of fat was transplanted per treated breast.


When evaluated after six months, the women had significant improvements in breast size and shape. On average, the treated breasts approximately doubled in size, with a "soft and natural?appearance and feel," the researchers write. Follow-up magnetic resonance imaging scans showed no cysts, masses or other abnormalities.


As they gained experience, the surgeons were able to perform the breast fat transfer procedure within less than two hours. There were no significant complications.


While the idea of breast fat transfer is not new, it has seen a resurgence in recent years, with several plastic surgery groups reporting good results. Drs. Del Vecchio and Bucky believe pre-transplant breast expansion is an important technical advance, providing increased space to be occupied by the patient's own fat. Patients also used the vacuum device for a few weeks after the procedure, which may act as a "splint" to help promote healing.


Further research will be needed to assess the results of the pre-expansion and fat transplantation technique?including not only the long-term outcomes, but also important safety issues. As reported in recent issues of Plastic and Reconstructive Surgery, there is some question as to whether breast fat transfer procedures interfere with mammographic screening for breast cancer.


In the meantime, Drs. Del Vecchio and Bucky believe their results?a two-fold increase in breast size, achieved in two hours or less?are a significant step forward in the use of autologous fat transplantation for breast enhancement. They conclude, "These results serve as a standard to objectively compare other techniques of fat grafting to the breast in the future."

Study Shows Women Battling Breast Cancer Now Have New Option for Reconstruction at the Same Time as Mastectomy

Cassileth One-Stage Breast Reconstruction offers customized, natural looking breasts immediately after one surgery.


For the first time, women can now wake up from a double mastectomy with customized breasts with virtually no scars. This new procedure was reviewed in a recently released study entitled ?One-Stage Immediate Breast Reconstruction With Implants? published in the "Annals of Plastic Surgery."


One in eight women will develop breast cancer in their lifetime. More women are opting for mastectomy with reconstruction. In 2010 more than 250,000 new cases of breast cancer were diagnosed in America. Last year, 93,083 reconstructive surgeries were performed. That?s an eight percent rise from the 78,832 in 2000. The majority of these women are choosing reconstruction with implants.


There was a time when women had to wake up from the mastectomy enduring the trauma of having no breasts. Then in ?90s a two-stage option of tissue-expander reconstruction became available. Today immediate implants at the time of reconstruction have become an option with limited use. The current immediate implant option offers women up to a 200-gram breast size reconstruction. So if a woman is a 750- gram breast size, she is forced to go smaller with the current one-stage procedure. The alternative is the painful process of the two-stage tissue expander procedure that involves expanding the skin with a temporary implant placed during the mastectomy followed by the additional fear and anxiety that a woman endures for the second surgery to place the permanent implant ?until now.


Dr. Lisa Cassileth, a reconstruction surgeon specialist in Beverly Hills, uses acellular dermal matrix (healthy donor tissue) to build an internal customized bra that can hold a silicone implant of any size. Dr. Cassileth sculpts the two-dimensional sheets of tissue into a three-dimensional bra-like shape that mirror the natural shape of the breast and then she tailors the tissue to the woman?s body-type, size and measurements. This eliminates the flat bump-like appearance that characterizes the current immediate implant approach. During the mastectomy, she works side-by-side with the oncologist to reconstruct the breasts and make tiny incisions. Post surgery, she conceals the incisions with a nipple reconstruction to erase any reminders of the mastectomy.


Dr. Cassileth conducted the study on 43 women and a total of 78 breasts. According to the study based on the judgments of patients and independent observers, this one-stage immediate reconstruction is superior in aesthetic outcome. Dr. Cassileth?s patients are even trading their older models, for newer, bigger, perkier breasts that they report are better looking than their own. This new option minimizes recovery time, costs, discomfort and psychological adjustment that accompany the current two-stage tissue expander procedure.


Survey studies report that an important factor in the choice for mastectomy is fear of recurrence. Studies also reported that the choice for conservation of the breast was due to a concern about the cosmetic outcome. This latest One-Stage Immediate Breast Reconstruction provides women for the first time with a solution to prevent breast cancer recurrence without sacrificing the aesthetics.
